For some HBAs and new kernels you find the information under /sys instead of /proc:
for Emulex HBA and Kernel 2.6 you can try:
$ uname -r
2.6.16.46-0.12-smp
$ cd /sys/class/scsi_host/
$ cat host?/modeldesc
Emulex LPe11002 4Gb 2-port PCIe Fibre Channel Adapter
Emulex LPe11002 4Gb 2-port PCIe Fibre Channel Adapter
Emulex LPe1150 4Gb PCIe Fibre Channel Adapter
$ cat /host?/state
